Did you know that nearly 9 out of 10 deaths and about two-thirds of the disabling injuries suffered by American workers in 2002 occurred off-the-job?1 Comprehensive accidental coverage is for union members aged 18 to 59 who want to avoid financial hardship and ensure financial well-being for their family. You have a choice between two benefit levels: Option 1 You and your family receive coverage for: - $50 a day for hospitalization (up to 500 days), and
- $500 a month for disability up to one year (after a 60-day wait period); and
- $50,000 for accidental death.
Option 2 You and your family receive coverage for: - $100 a day for hospitalization (up to 500 days), and
- $1,000 a month for disability up to one year(after a 60-day wait period); and
- $100,000 for accidental death.
Even if you already have life insurance, it may be essential to supplement it with a plan that pays you while you are hospitalized due to an accident. CAP is an all-in-one solution, combining three different types of coverage, including: - Hospitalization due to an accident. Your coverage begins the first day you're in the hospital and continues for up to 500 days of being in the hospital - no waiting period necessary - no matter how minor.
- Disability due to an accident. After 60-day qualification period, the policy pays you, in addition to your hospitalization benefit, a monthly benefit for up to12 months if you are Totally Disabled and unable to perform the activities of your usual occupation as a result of the accident. Your disability payment cannot exceed 70% of monthly wages.
- Accidental death. Provides financial relief for any accidental death - including a death that occurs up to a year (365 days) after the original accident which caused the fatal injuries.
It's coverage you and your family can rely on: - Guaranteed to be issued - no health questions or medical exam required to enroll.
- Premiums do not increase and benefits do not decrease with age - you won't pay more for your coverage, or lose any of your benefits, until the policy terminates at age 65.
- Strike waiver of premium - we will pay your premium for up to one year if you are involved in a union-sanctioned strike or lockout. May not be available in all states or for all unions.
- To better accommodate seasonal workers - whose job schedules change throughout the year - eligibility is based on an average of 260 hours worked per calendar quarter.

- Spouse or domestic partner can purchase coverage separately even if the union member does not apply.
